You are eligible to participate if you have ever played soccer or tackle football, at any level, and are currently age 40 and over.

HITSS is a fully-online confidential survey, including: questions about sports participation, repetitive head impact exposure and concussion history, and medical history; computerized memory and cognitive tests; and behavior and mood questionnaires. Completing the entire HITSS survey takes approximately 60-90 minutes, and can be completed at any time and over several days if desired.

Participants who complete the entire HITSS survey, including Parts One and Two, will be entered into a drawing for the opportunity to win a $500 gift card. Winners will be drawn at the end of each month and odds will be determined by the number of people completing monthly.

You may enroll into HITSS and complete most of the survey on a mobile device. The computerized memory and cognitive tests in Part Two can only be completed on a laptop or desktop computer, thereby we ask you to log in on a computer to complete Part Two.
